Intake Coordinator - Full time
Summary:The Intake Coordinator is responsible for coordination of incoming faxes, referrals and data entry for intake department. Support the process of scheduling clinical staff by timely processing and distribution of patient information in the clinical area.Responsibilities:o Enters referrals from liaisons and intake into the electronic medical record timely and accuratelyo Reviews incoming referrals for all home care requirementso Acts as liaison to Admission Coordinators and to hospital liaisonso Communicates referral needs to clinical staff for review in a timely manner o Effectively completes necessary follow up with referral sources, physician's offices, patients and staffo Scans all clinical and referral documentation accurately into electronic medical recordo Enters non-admits into electronic medical recordo Maintains physician data base with updated license and PECOS statuso Runs Medicare Eligibility on new and pending referralso Reads Medicare eligibility correctly and completes required follow upo Annually completes Compliance Education and understands applicable policiesQualifications:o Education: High School diploma or GED. o Experience: A minimum of 2 years' experience in data entry. Preferred Position Qualifications: o1 year data entry experience in healthcare.o Prior intake experience a plus 1 year data entry experience in healthcare.
